Terms and Conditions

 

My commitment to you

  • I am a qualified Chartered Physiotherapist registered with the Health and Care Professions Council, and I will examine and treat you in line with the Chartered Society of Physiotherapy's strict code of practice. Following assessment I will discuss further management with you and answer any questions that you may have regarding treatment. You may decline any of the treatments proposed, without prejudice.

  • Your assessment and treatment will be in the privacy of either my treatment room or your home. Treatment location will be agreed prior to assessment. Please note that travel to your home will incur mileage costs - please contact me for details.

  • Appointments are scheduled according to your needs and my availability.

  • A letter is normally sent to your GP or the person referring you unless you request otherwise, and you will be asked to sign a form consenting to this. Anyone may book an appointment.  A referral from a GP is not necessarily needed, unless a medical insurance claim is to be made for the cost of the treatment.

 

Your commitment to me

  • You will provide any information needed in order that I can effectively assess and treat you (this usually is covered by your responses in the assessment).

  • Arriving late for an appointment may result in you receiving a shorter treatment. Full payment will still be required. 

  • Payment (preferably by card) will normally be made at the end of each appointment.

  • Insurance clients will be asked to settle their bill at the time of treatment, and then get reimbursed by their insurance company.

  • Please note that 24 hours notice is required if you wish to cancel an appointment and avoid a cancellation fee. If less than 24 hours notice is given a fee will be owed (50% day before, full fee on day or no-show), and further appointments cannot be booked until this is settled.
